AntiCoagulation Self-Monitoring Alliance: Patient Choice Awards

To celebrate the excellent work that healthcare professionals and carers are doing around the UK, ACSMA is holding its first Patient Choice Awards.
There are more than 1.2 million children and adults on long-term warfarin in the UK, many of whom are receiving excellent care and support in coping with their condition. The dedication and professionalism of the healthcare professionals working in this field play a vital role in getting the best outcomes for patients. In addition, carers and family members have an important role in supporting people who need anticoagulation.
ACSMA need your help to find those who are really championing the cause of people on long-term warfarin. We need you to nominate an individual or team who has gone above and beyond the call of duty. You could nominate your: GP/consultant, nurse, anticoagulation clinic, carer, family member, etc. If you would like to nominate someone please download and complete the ACSMA awards nomination form Please complete using adobe acrobat or it will not work. Otherwise simply complete and post/email it to us. The closing date for nominations is Thursday 17th April 2014.
A panel of judges will sift the nominations and identify three Patient Choice Award winners for each of the following categories: carer, healthcare professional and outstanding achievement. The short-listed candidates will be invited, and those of you who successfully nominate them, to attend a high profile awards ceremony in June 2014 hosted in Parliament by Virendra Sharma MP.
